Par Pharmaceutical, Inc. supports educational grants, sponsorships, charitable contributions and Continuing Medical Education ("CME") requests (collectively, "Grants") which enhance the practice of medicine.

In order for us to be able to consider your request for a Grant please complete the Educational Grants Request Form and the Educational Grant Agreement.

The Educational Grant Agreement must be completed in its entirety and signed by an appropriate representative of your organization in order for your request to be considered.

The Request Form and Agreement should be faxed to (201)-802-4280 or scanned and sent by email to compliance@parpharm.com.

Upon receipt your Grant request will be reviewed by Par. Upon completing our review you will be notified whether Par is able to provide financial support for your Grant request.
